Living with Long Term Conditions
We offer a range of options for people living in Croydon who are suffering from a long term condition, including: COPD, coronary heart disease/heart failure, diabetes, skin conditions like psoriasis or eczema, irritable bowel syndrome, chronic fatigue syndrome, fibromyalgia and chronic pain.
This includes CBT, Guided Self-help and Groups.
We work with pulmonary and cardiac rehabilitation services in Croydon to provide a holistic model of care for those with COPD/cardiac problems and are also integrating psychological well-being into other community health services.
We work from a variety of locations across Croydon, including Croydon University Hospital, and other community health clinics. Limited home visits and telephone support can be arranged if individuals are too unwell to attend a clinic.
Long term conditions group:
This is a 6 week general group for people living with a chronic health condition. It focuses on some of the stress and distress of living with a long term condition and tackles issues like pacing, managing health worries and dealing with the losses that come with ill health. This runs 4 times a year.
