Long Term Conditions Team
It is very common for people who have been diagnosed with a long term condition to feel low, frustrated or worried at times. Living with a long term condition can affect a person’s mood and how they feel can impact their health condition.
Our Long Term Conditions team can support individuals improve their overall wellbeing. They can learn to cope better with the demands of managing their condition which has a positive impact on their mood and health. We work with a range of problems including: coronary heart disease/heart failure, COPD, diabetes, skin conditions like psoriasis or eczema, irritable bowel syndrome, chronic fatigue syndrome, fibromyalgia and chronic pain.
How can we help?
We offer a range of options for people living in Croydon. This includes CBT, Guided Self-help and Groups. We also offer online support through Silvercloud which is tailored to specific conditions.
We work from a variety of locations across Croydon, including GP surgeries and other community health clinics. Limited home visits and telephone support can be arranged if individuals are too unwell to attend a clinic.
How can I access this help?
You can access this support through making an appointment with your health professional or GP who will be able to send us a referral form.
The Long Term Conditions team requires a referral form emailed to us at CroydonIAPT@slam.nhs.uk or faxed at 020 3228 2955. When using the form please include any relevant additional information, such as risk or medical history, as we may not be able to process the referral otherwise. You can download the referral form here.
Once a referral is received, we will send a letter to the patient asking them to opt in to our service by calling 020 3228 4040. Please note we are an opt in service so unless you indicate the individual is unable to opt in we will wait for them to contact us after receiving their referral. Please give them one of our leaflets and advise them they need to call us to book their assessment appointment after receiving an opt in letter.
